HIPAA Notice of Privacy Practices

This notice describes how protected health information (PHI) about you may be used and disclosed, and how you can get access to that information. Please review it carefully.

Uses and disclosures for treatment, payment, and operations

  • Treatment: your provider, care team, and the dispensing pharmacy use your PHI to evaluate you, prescribe, dispense, and follow up.
  • Payment: PHI may be used to bill you or a payer for services and medication.
  • Health care operations: PHI may be used for quality review, licensure, training, and administration.

Other permitted disclosures

  • When required by law, court order, or public health and safety reporting.
  • To business associates under written agreements protecting your PHI.
  • To persons involved in your care, when you agree or when permitted in an emergency.

Most other uses — including marketing that involves PHI and any sale of PHI — require your written authorization, which you may revoke in writing at any time.

Your rights

  • Inspect and obtain a copy of your health records, generally within 30 days.
  • Request an amendment to records you believe are incorrect or incomplete.
  • Request restrictions on certain uses and disclosures.
  • Request confidential communications at an alternate address or number.
  • Receive an accounting of certain disclosures.
  • Receive notice of a breach of unsecured PHI.
  • Obtain a paper copy of this notice on request.

Our responsibilities

We are required by law to maintain the privacy and security of your PHI, to follow the terms of this notice currently in effect, and to notify you if a breach compromises your information.

Complaints

You may complain to our Privacy Officer using the contact details on this site, or to the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
